L2 Background Check-Demographic Information

All program sponsors and program staff must successfully pass a State of Florida level 2 background check in accordance with federal and state laws, as well as UCF Policy 3-011 Background Checks. Eligibility to work supervising minors will be determined by the Florida Department of Children and Families.

The below information is required to initiate the Level 2  background check request with the Department of Children and Familes for the State of Florida. 

Once the form is completed and reviewed by the background check team, you will receive an email with instructions on how to schedule an appointment to have your fingerprints taken.
